If you or someone close to you needs that little extra support after being discharged from hospital, then our Reablement Services could be for you.

At Candlelight Care, we run a specific Home First service in conjunction with discharge coordinators and reablement teams at both the Royal United Hospital in Bath and Yeovil District Hospital.

When a patient no longer needs to stay in hospital and can be safely discharged, we are there to help with further rehabilitation/reablement in the peace of their own home.

We deliver Pathway 1 and we can provide short-term care for you or your loved one whilst they are recovering from their hospital stay. Roughly, this Pathway tends to last for around 3 weeks.

The hospital will discharge you or your loved one onto one of the Pathway’s below:

Pathway 1 – Upon Discharge, your needs can safely be met at home with the help of a Home First team.

Pathway 2 – You may be placed into an interim rehabilitation placement, that are intended to enable a safe return home once you or your loved one is able.

Pathway 3 – You or someone you love is unable to return home initially as the care requirements are more complex. You or they may need a permanent change to their care.

Working with the hospital, we will support you at home with your recovery and help you rebuild your independence during your reablement. Plus we will encourage, prompt and assist with rehabilitation exercises to get you back on your feet. All of our Reablement team have been trained by fully qualified and experienced hospital staff, such as Physiotherapists and Occupational Therapists.
